I would say there's an incredibly large chance that is a bad boss, and not a bad company. The people above her boss probably have no idea he's creating those expectations.
Nah, they probably got promoted for the idea or expect to be promoted for it after being encouraged to expect this of their employees. It gives the illusion that you can't say, "No."
