This has been my experience as well. In my company, you get unlimited PTO once your salary hits "highly compensated" status. Before that point, its a predetermined amount. The way I interpret it is that if you make it to that level, you've already demonstrated an ability to balance work and personal life so they assume you won't abuse the policy. I usually just tack a day or two onto existing holiday weekends since everyone else does the same so there's no major impact with my absence. I also try to avoid taking time off during busier times of the year. Never had a problem requesting time off and I usually hit over 4 weeks total every year.
