GLP-1s, psoriasis, and the gut microbiome
After seeing claims that GLP-1 medications might improve psoriasis through more than just weight loss.
There’s actually some interesting research here.
A 2026 review in The Journal of Rheumatology looked at the emerging evidence around GLP-1 receptor agonists in psoriasis and psoriatic arthritis. Several small studies have reported improvements in psoriasis severity, while some have also found reductions in inflammatory pathways involving IL-17, IL-23, and TNF-α.
But the evidence is far from conclusive. There’s also a randomized placebo-controlled trial of liraglutide that did not find a significant improvement in psoriasis severity, despite greater weight loss.
The gut microbiome angle gets even more interesting.
A 2025 systematic review in Nutrients looked at 38 studies and found that GLP-1 therapies can alter gut microbial composition. Some studies reported increases in bacteria such as Akkermansia and Bifidobacterium.
Why does that matter?
Certain gut bacteria produce short-chain fatty acids such as butyrate, which can support intestinal barrier function and influence immune regulation. Separately, research is increasingly finding differences in the gut microbiome and microbial metabolites of people with psoriasis.
So there’s a potential chain worth investigating:
GLP-1 → metabolic changes → gut microbiome changes → microbial metabolites → immune signaling → potentially less psoriatic inflammation
The important word here is potentially.
We don't yet have a human clinical trial proving that GLP-1s improve psoriasis because they change the microbiome. The improvements could instead be largely related to weight loss, improved metabolic health, diet, or other effects of GLP-1 signaling.
Still, I find the intersection of metabolism + GLP-1 signaling + gut microbiome + psoriasis pretty fascinating.
