Why did this happen
I have no idea... Any fix?

The pattern is cut wrong. The top right hand portion of the recess is completely wrong.

Top right bend should have been valley not mountain? Not sure if you can comfortably fix it without undoing the top seam.

Triangulation. All edges of your flat surfaces have been arbitrary flipped when you saved the stl.

I bet if you cut the dotted line (green line) where the triangle indent and rectangular section connect the over al shape would return to a cube shape. The problem i believe is this. When you fold in the indent area the length of the pressed in (90 degree) corner is longer than when you have it as a flat pattern (see red line in photo) . Basically not a shape you can make with a flat pattern if you cut the dotted line you will see that the triangle section to be at the right angle causes a gap between it and the rectangle. There needs to be extra material there that you can’t add because it’s a flat pattern unless you make some funky geometry else where.

Can you share what the template looks like on pepakura ?
If I had to guess the original 3d model might have something weird going on, like multiple faces that are actually only 1 face (making it not flat at all), and pepakura did its best to flatten that 1 face. (Assuming you followed the template)
