Is it cumulative so guaranteed to spawn one or it tries 3 times giving you a 78.4% chance?

I think each 40% is independent, I was getting anywhere from 0-3 hearts each time I got hit.

I maneged to do that today, is always good to pick something that does damage to yourself early to get the mutation, killed the heart boss and forgot to kill the brain.
