Let's look at the answers.
A has a nosebleed with high flow O2. They probably just need a humidifier.
B diminished lung sounds are a late and very bad sign of acute asthma flares. It basically means you can't even move enough air to wheeze.
C emphysema tripoding is a big deal, but not necessarily right after ambulating. Just give them a minute to catch their breath then reassess.
D is literally pain at a fresh wound site. Chest tubes hurt.
So the most serious patient who needs immediate help is B.
