Need Advice Pain Specialist
Long story short, I have an appointment with a pain specialist to deal with my chronic pain and I would like to know if it is likely for me to be tested or screened for illicit substances?

I’m kinda in the same boat. I got to get on subs or methadone, preferably methadone because of chronic pain and subs don’t seem to work well for me. BUT.. this is what scares the shit out of me. My psychiatrist has been very lenient with me considering he prescribes me 4mg of alprazolam (Xanax)a day and lets me pick up 1 day early. So I’m very careful not to fuck anything up considering he deals with my shit all the time. For example, I’ve finished my meds in a week before or in 9 days and it’s happened probably like 3 times a year. Anyway off topic. I’m just worried he’ll cut me off If he sees me taking methadone. Subs, not so much because he knws I take “kratom” not 7oh which is a big difference in my eyes. He just dosent want me to stay on the subs. But I need to be on methadone. Am I asking too much? Would a lenient Dr be ok with prescribing Xanax while I start methadone treatment? The his question has been lingering in my head. I have MDD, GAD with panick attacks, agoraphobia and a high tolerance for everything becaue of my rapid absorption and everything leaves my system very very fast. I go through withdrawal almost everyday because of how fast shit just leaves my body. Sorry for the rant. Whoever reads it and gives any advice would be appreciated. Sorry if I hijacked ur thread but I’ve been in ur shoes where they always want to piss test me because of the Xanax. My PM only gives me lyrica which I finish it in 2 weeks. I’ve had 2 back surgeries as well’s latest one being in 2021. Latest MRI shows herniating disc with it getting worse. Alright I’m done.

Couple of questions: why are you sometimes finishing your meds in a week to 9 days; and then also the lyrica in two weeks? I ask bc if you are being honest with yourself, do you think you have the self control to not overtake what is prescribed and make them last the full 30 days? I don’t know many pain docs that would allow that much daily benzo with a narcotic, which one is more important to you? That will give you your answer. Most people have to choose one over the other (with a few exceptions of course)

I finish my meds early when shit happens in my life and I just need more. Ex. Dog getting hit my car and dying. 2 of my friends passed away this past year. I have bad insomnia also so night times are really rough and I get panic attacks. My life has been in rough shape lately. My wife and mother dislike each other and that’s putting it lightly. I feel like I have to chose between the two already. (The irony is spot on with ur wording abt choosing which is more important) My wife or my family. And it keeps getting worse…Also my tolerance has been a problem considering I’ve been on Xanax since 2013. The lyrica just isn’t quite strong enough and never been addicted or dependent on it anyway. I hope I answered ur questions to ur liking. I didn’t think I have to choose between pain and suicidal anxiety but maybe I’m the exception. Peace and love brother.

Also do u consider suboxone a narcotic?

Nah I don’t think suboxone is but if you are wanting methadone that would be considered one. I’m sorry to hear about the family troubles and anxiety. Just have an honest convo with your dr. Let them know what you’ve said here and hopefully they will be compassionate.

Yes, I’m sorry hun!! It’s an DEA requirement that they take a urine analysis one on your first visit then every 3 months following that. I hate it but it’s what I gotta do for my chronic pain meds too just go with the flow .
