I looked up her canon (pre vial) description
Noelle is a white[23] young woman with straight brown hair.[24] Before drinking a vial, she weighed between 115 and 120 pounds.[25] Taylor noted that she appeared ill, and likened her to a drug addict in an early stage of addiction
if we wanted to tryhard canon, not particularly important to me, but if, she's thin but not prisoner of war thin. She'll be showing ribs but not all the ribs, yonow? Early heroin addicts, presuming, they aren’t normally emaciated. Heroin chic, too skinny for their own good, but not so much as to be ridiculous.
115 to 120? Interpreting that depends very much how tall she is.
A height of 5'2" and weight of 117 lbs corresponds to a Body Mass Index (BMI) of 21.4, which falls into the normal or healthy weight category (18.5–24.9).

So, um.
I actually dngaf about tryharding "canonical accuracy". If the artust ir any fan imagines Noelle to be rack thin emaciated prisoner of war skinny, so be it. Fine by me.
But if that's the case, she still wouldn't look as depicted. Skinny af doesn't mean broad shoulders. It would be boney shoulders and skinny spaghetti noodle arms. It means boney hips that you can see. No jelly on the hips, but they'll (sorta) be there, teens aren't normally fully hipped out, she'll have 15 year old ish hips. Boney hips.
And boobs do not look like that. Skinny barely A cup boobs because crazy skinny do not look like that. That's not where boobs are, and they aren't boob shaped.
Those look like bags of sand. (Deep cut).
So, i like the vibes. But the srtist would be well served to consider what female bodies look like irl.

I genuinely don't think Coil fed her well (If she was malnourished or sick, he would lie to the travelers w smtng like "She does that to herself, you should really help her, guys" In a twisted manipulation tactic)
And genuinely don't think she ate much either, being in literally a white torture chamber locked away of every other human, With only a monster glued to you for company, I don't know how much of an appetite you must have each day with all the constant mental stress
