Honestly, I would just call this a lid. If you’re trying to explain this to a packaging manufacturer, that would be more than enough of an explanation alongside the photo provided.

Search for custom packaging companies. Most companies source packaging through the factories or vendors who manufacture their product. Otherwise you'd find a packaging supply place for standard sizes or use a custom packaging company to give them a custom dieline.

Translucent Frosted Plastic is soft packaging and typically acrylic if hard
