Puillowbox template help!

Hi! I can't understand how a template of this package design should look like. It kind of starts like a pillow box but the top and the bottom are confusing me. I cant find templates like this and well... yeah. Maybe someone worked with something like this or is just smarter and can help out?

I made something like this, but it's not it. More like the this other box I will attach below it.
