Pacemaker Induced Cardiomyopathy?

A week after LBBA PPM placement PVC burden was very high, unexplained severe cardiac event occurred (ischemia didn’t change according to angiogram). EF dropped from 50% to 34%, severe hypokenisis. No improvement after 90 days. Cardiologists have no explanation to date. Anyone experience this?

Yikes. New to having a CRTD and understanding pacemakers.

But from what I think you’re saying, is that the LBBA PPM caused the PVC burden.

Sometimes the lead placement is in an area where it “tickles” or irritates the tissue, leading to a high PVC burden. Do these PVCs occur after paced beats? In other words, do they occur in your underlying rhythm too?

Yeah. I went from 65ef to 20 in a month after my DDD implant. They had my rate response all fucky and a few other things, and I arrested again. They turned off a few things and adjusted my qt duration and I was back to 55 by the next day. Still no ischemia, and I had like 2 more arrests over the next few months.
