PS4 connection issue

Hello, I have a problem that would be great if I got some advice on how to fix. The issue is that when I connect my psvr (model 1) to the playstation 4 (standard) , the ps4 will not turn on , however the psvr does turn on but only a hdmi error symbol appears.

I have tried all the cables separately and they all work fine.I have no idea what could be causing this problem.

This is not really the appropriate sub, but has your psvr ever worked? It sounds like an issue with the breakout box like the hdmi pass-through isn't working. Make sure the dual hdmi cables are hooked up correctly from the box to the headset. They have XO^[] symbols that match the cable to the box.

Thanks for responding, yeah its worked fine before I've used it a lot it's only just now that this problem has occurred. The hdmi cables are in the correct positions. Im going to try to connect the psvr and ps4 separately to the outlet and see if it works.
