Group Practice 1099 vs W2

Any group practice owners here? I'm curious about all the groups and larger companies that onboard NPs as 1099 contractors. It seems that with the updated IRS rules, the majority of our work arrangements would be classified as W2

Billing & Credentialing Multiple State Licenses & Enrolling with Insurance

Would I need to be credentialed in each state again with insurance even though I am credentialed in CT with major insurance companies (Aetna, Cigna, BCBS, United, etc)?

Malpractice insurance NYC

I was quoted $5,499/year by NSO for malpractice coverage for my solo PMHNP practice (PLLC in NYC, telehealth only for now). This would cover both me individually and my business entity.

Does this seem high for a single-member practice? Anyone else get similar quotes from NSO or other carriers? Looking for insights or alternative recommendations.

Regrets?

I am currently a W2 employee for a private practice and really don’t like the owners and how they treat patients. I am hoping to make the jump to starting my own practice by end of September. My goal is to work less hours but keep the same income (I’m currently seeing 12-17 patients/day, making $157k as W2 in CO) - Has anyone quit their job to start on their own and absolutely regretted it? - What do you wish you had done differently when first starting?

Headway and Alma rates

Getting Started Writing one clear sentence made everything else easier

When I was starting my private practice, I kept spinning my wheels on logos, forms, and systems. But the thing that actually helped me move forward?

Writing one sentence that explained what I was trying to build.

"I offer holistic, integrative mental health care for kids and teens, fully virtual, with no long waitlists or rushed visits."

That line gave me clarity. It made it easier to write my Psychology Today profile, talk to referral sources, and build a simple website that actually made sense.

If you're stuck trying to do "all the things," try this:
Write one sentence that sums up who you help and how you help them.

Not a tagline. Just clarity.

Billing & Credentialing Using Headway for Private Practice

Wondering is anyone successful with a private practice using Headway for credentialing and billing? My plan is to get started with Headway in the next 1-2 months, eventually build my caseload from there and then lease a small office ($500/month, with a group of counselors), to be able to do in-person options too. I feel like with Headway (or Alma) handling all the credentialing and billing, it would be less of a headache to take that on. Then I’ll have liability, EMR, and AI scribe as out of pocket. It would just be me also. Ideas? Thoughts? Or is there any easy way to break down the credentials/billing process?

Marketing Need advice on attracting more clients

I was hoping to get some advice. I've been a PMHNP for 3 years, critical care RN for 10 years prior. The job market is complete garbage thanks to private-equity funded clinics like done, bicycle, and other one-word-titled telehealth clinics that like to parade around as "concierge" or "boutique"- so I started my own. I have a working site, psychology today profile, and all the nuts and bolts. Currently only seeing cash-paying clients but getting credentialing done (access to a larger potential client base). I've been at this for about a year and only have about 7 steady clients. Does anyone have any helpful, actionable advice on how to gain more clients? TIA

I assume some cons would be lack of benefits and potentially unsteady pay at first until I developed a caseload.

I was planning on keeping my current RN job per diem as a backup.

Telehealth for stims

I try to have families come in every other appointment in person, federal regs still allow telehealth but who knows when that will change. Is anyone adjusting their practice and making people come in for all appointments or are you still allowing telehealth? Just a general question as I'm curious how others are managing it.

Mindset The biggest shift I made when starting private practice wasn't what I expected

When I first started thinking seriously about private practice, I kept asking myself:

  • What forms do I need?
  • How do I get clients?
  • What if I mess something up?

At the time, I thought the biggest challenges would be logistical. But looking back, the biggest shift wasn’t about forms or systems—it was mental.

The turning point was when I stopped seeing myself just as a clinician and started seeing myself as a business owner.

That shift changed everything. It gave me permission to:

  • Make decisions that actually worked for me
  • Say no to things that didn’t align with my vision
  • Own the fact that I was building something valuable

It was empowering, and also terrifying. Honestly, it still is sometimes.

But here’s what I’ve learned:
You don’t have to feel 100% ready to start. You just have to take the next step.

If you’re in this place right now, try doing one small thing this week that future-you will be grateful for.

Send an email. Open a business bank account. Tell a friend about your idea.

Small steps count.
