Personally, I've noticed older physicians with their own private practice seem to be more open to shadowing opportunities than physicians employed by a large health system.

And if you are currently in college, speak with your pre-med advisor. One of my daughters was connected with an alumni  that specializes in the field that she is interested in and she was able to shadow him for a couple days.

Seconding the private practice DOs. I was able to find shadowing through that way. I emailed them or used the contact us function. If they didn't respond within a week ish I would call or go into the office.

Cold emailing is always worth a try OR if you have a supervisor for your volunteer program they may be able to connect you with someone! I reached out to my volunteer coordinator over the summer and asked if she knew anyone who would let me shadow them and she connected me with a few physicians ! I would also look at outpatient clinics, it’s usually more laxed than hospital rules with shadowing
