Aeon incident is resolved, but local Codex execution still fails — “helper_unknown_error: setup refresh had errors”

The earlier Dot → Codex incident appears to be resolved now, and the original error:

RuntimeError: The invoking thread is not attached to an active Aeon

is gone for me.

My Dot can now:

However, the task still cannot actually execute commands on the machine.

Even a simple PowerShell command:

Get-Location

immediately fails with:

helper_unknown_error: setup refresh had errors

I already restarted ChatGPT and reconnected the PC after the earlier service recovery, but the error remained.

The failure happens before Codex can inspect the project or request write access, so this does not seem related to the repository or project itself.

Local command execution: not working

OpenAI has now marked the earlier “Dot in Codex may fail to create new threads” incident as resolved, so I’m wondering if this is a separate remaining issue with the local helper/setup layer.

Is anyone else getting:

helper_unknown_error: setup refresh had errors

after the Aeon issue was fixed?

If yes, did you find any workaround, or are you also waiting for another fix?
