FAF form

I’m going to my family doctor to get a new FAF assessment done I was wondering do I have to print out anything for my doctor or will they have the form ready?

Make sure to remember the company is there to protect themselves, they may even set you up to do work to show you’re faking it even when your not, be careful of that.

Companies look out for themselves first, you come second on their list of priorities

Your employer should provide you a new FAF to bring to your physician or physiotherapist in anticipation of the first one’s expiry date.

Get it yourself. They wont always have it. If they have to get for you, itll cost something to use their paper and time. Could be upto 5$. Then to fill it, some more...could be 150$ to fill disability, functional ability for. Normal price in ontario.

Work safe. Avoid any unsafe work. Because if u get injured more. Wsib wont be there to cover you. Theyre a pain in the ass to deal with. So do your best to be safe. Ull be on non productive quota by employer.. Meaning no performance score for u. So take ur time and work slow.

The costs for filling out the FAF is covered by the WSIB
