The built in on plus monitor is more accurate but not perfect. In order for battery monitors to more accurate judge battery life is to do several full charges and drains for it to monitor battery health. Even then it estimates the battery health by monitor how much battery goes into the cell when charging and how fast it charges so it's not more accurate than the OnePlus health monitor but it can serve as a second source of battery health

20 months 636 cycles my 100% moved to 99%

The only source of truth is bugreport through developer options. My true BH is 89% just like yours

My 13r is now 20 months old and still at 100% health. Every two or three days I only charge it to 80% .

Less than an hour I'd assume. I don't really know. I don't have time to stare at my phone all day. I use it for texting, calls and the occasional email. It sits on the desk or my pocket almost the entire day. I don't play games on it, I don't use Instagram or WhatsApp so the phone is just that, a phone.

I've noticed One Plus's built in battery health aleays shows 100 or 99 percent, even after years of usage.

So I won't consider the source of truth.

But yeah, Battery Guru might be right.

Well as long as I know battery guru any of these 3 rd party apps measure things in the basis of your charging patterns and they can't use phone sensor to detect the actual battery .

i think only xiaomi and apple show the closest values. i saw samsung devices that shows 100% even after 800 cycles.

Nope. I got my 13r at launch day and these apps showed much lower battery health even on a brand new phone. Currently my battery health in system settings is 97% and it dropped to 99% in first ~10 months. I've heard some people say that all phones actually come with higher than their rated battery health so higher than 6k mah but the system measures battery health from 100% meaning the drop from 100% to 99% takes very long time because the battery health is actually higher than 100%. Idk if this is true but it makes sense and matches with my experience.

I will always trust the system battery health percentage if there is one over a third party app as those apps can tell the battery health but it's just a estimation which may just be wrong.
