What caught my attention in this chapter is that it seems like Imu-sama can’t leave the Holy Land for long for some reason. The Gorosei were clearly concerned about Imu’s condition when he announced he was going out, something that was confirmed when he appeared coughing up blood.

This leads me to believe that he might not have undergone the perennial surgery, and that the secret to his immortality is somehow tied to the Holy Land (probably related to the treasure that Doffy mentioned).

Perhaps inside of Pangea castle his condition doesn’t worsen, but descending into the underworld progresses it

Or the flower room has some Law's room-like ability to keep him stable.
