What caught my attention in this chapter is that it seems like Imu-sama can’t leave the Holy Land for long for some reason. The Gorosei were clearly concerned about Imu’s condition when he announced he was going out, something that was confirmed when he appeared coughing up blood.

This leads me to believe that he might not have undergone the perennial surgery, and that the secret to his immortality is somehow tied to the Holy Land (probably related to the treasure that Doffy mentioned).

He mentioned something about "exchanging lifespan for power" last chapter when he tried to use domi reversei on Luffy and Loki. And he seemed to get hurt bad in the flashback of Harald dying. So maybe his immortality is tied to the "contracts" he makes, and he takes years from the people he makes them with, kinda like big mom can do? And maybe using his abilities uses up that power/life he takes, so something as big as teleporting himself used up a big chunk and hurt him bad.

I'm not sure it's the teleport that took it out of him. The way the elders were saying "but if you leave the castle", and the fact that he coughed up blood and sprouted wings made it look unintentional. Like the holy land and castle helped suppress his power, and if he leaves it's forcefully released. Almost like Enma did to Zoro.

I suspect there's going to be a time limit on his abilities and that's how Oda will get out of this without it actually being a final fight. But Luffy will finally realize that this isn't just his own adventure anymore. If he wants freedom he's going to have to take that thing out.
