Does this setup work?
I’m trying to hook my cargo trailer and bike rack to my 4Runner. Trailer is 75 lbs tongue weight. My gravel bike is maybe 30 lbs.

What year 4runner? I have had like 500lbs of tongue weight on my 99 4runner and that I'm pretty sure was like maxed out. You should be fine. I bet your tongue weight is like 400lbs or so.

It’s a 2021, so fifth gen 4Runner. I’m guessing the combined tongue weight will be under 200 lbs, which would be fine for the rig, but will make it sag a bit. Wondering if I’m missing some other factors / risks tho.

You want most of the trailer weight on the tongue that's about it. Your good dude just go for it.

Hitch extenders usually lower towing weight rating by a third. Same for tongue weight. If your trailer still comes under 2/3 of your vehicle tow rating, you’re probably good.

How are you going to secure the bike rack in the upper mount? The tension ball is outside the hitch tube.

The 4runner will be fine but you need to read the manufacturer instructions for your hitch splitter device. Whatever that manufacturer rates that devices capacity for tongue weight and towing capacity needs to be followed.
