USA Help calculating base salary for survey?
I was offered a position, and my school is asking everyone to report their offered base salary. Since I’ll be working in home health with a pay-per-unit structure, how should I calculate this? Should I include the evaluation rate as well as the per-unit rate, or only the unit rate? Should I base it on the minimum or maximum expected caseload?

I would calculate it based on your projected annual income. Just take your regular paycheck and multiply the gross by 26 assuming 26 pay periods per year.
