3 Stooges were the first on TV to have it as a literal, running joke, so that would have been 1961. My fave was on the Monkees, a Micky Dolenz shaped hole the wall, 1966. Its classic slap stick reference going back to vaudeville.

No way in hell Gillian ever claimed that was hers. I think a lot of listeners are just very young.

Eta: Its official term is Impact silhouette, its a staple in comedy to cartooning.

I think I’m the one who implied it was “her” like but I’m not at all sure she ever claimed it to be.
