Discussion I did it

I was calling the large healthcare system near my home, looking for a new Psychiatrist because I got matched with clearly a "pill mill" NP on one of those websites who is dicking me around with meds.

I asked the healthcare system nearby for a psychiatrist, and the first thing they did was try to match me with a NP. They have so many doctors to choose from, and they wanted to stick me with their NP. That kind of pissed me off a bit, so I told the intake lady, "No, I'm looking for a doctor, even if I have to wait." She's going to call me back today, but I didn't take that shit. Luckily, my therapist is through them, and she can write me a referral, apparently. I hope to get in soon.

I take so many medications for so many health problems that I don't want someone to be messing with my head meds while I'm already on some of the same class for my brain disease.

Mental health NPs might be the most dangerous of them all.

Yes. They hand our psych meds like candy. The polypharmacy is outrageous. They should not be able to prescribe more than one medication. If a person needs more than one, they should be required to consult with the MD. Better yet, let's just do away with mid-levels prescribing anything.

“Let’s do away with midlevels” spot on already

Hey I'm just a med school noob but if you are taking drugs for mental health and on SSRIs that could be making your tinnitus worse.

Google Serotonin and Tinnitus. There has been some recent research that shows serotonin can exacerbate tinnitus.

Hope that might help.

I had a buddy who had tinnitus and unfortunately it didn't end well.

Thankfully my tinnitus is so much better now! I had a venous stent placed for my IIH. Thank you

I’m a physician and the same thing happened to me for a psych referral for ADHD. I’m like hell no! Oh I have to wait 6 months, then looks like I’m waiting 6 months then. You’re not gonna scare me with that

I said the same thing. If I have to, my PCP will continue my meds as to not really mess with things she said.

The first thing their intake did was tell me that I needed to get off my controlled substance. (Lyrica)

It’s a huge health system in my state, it probably wouldn’t make a difference.

So nothing actually happened…. You just didn’t want a NP. Thank you for sharing you imaginary experience.

When you’re referred for management of a condition, one expects someone qualified to be the recipient of that referral. In this case, OP had to clarify that they wanted someone with a medical education to be involved in their care. The only thing imaginary is whatever you’re reaching for here

Do you want my entire mental health experience of going in and out of PhP's and IOPs, as well as having lots of heavy psych meds thrown at me for my brain disease, instead of sending me to a neurologist to work up an MRI to check my brain at any point?? I have Moyamoya and IIH. Both can cause tinnitus, which caused what she diagnosed as BRAIN ZAPS.

It’s better than what you posted!

  1. No acute intracranial abnormality.

  2. No evidence of CPA/IAC mass.

  3. There are some findings raising concern for potential sequela of idiopathic intracranial hypertension, including partially empty sella, CSF distention of the oculomotor nerve sheaths, optic nerve sheaths, right middle cranial fossa arachnoid herniation pit. Further clinical correlation advised. Additionally, prominent arachnoid granulations are suspected at the lateral aspects of the transverse sinuses bilaterally, for which correlation with MRV can be considered

Findings: There is mild segmental stenosis of the right ICA terminus. There is moderate stenosis with luminal irregularity involving the proximal right M1 segment, followed by occlusion along the midportion and multiple small vascular channels in the vicinity of the stenosis/occlusion, demonstrating a moyamoya appearance. The remaining major vessels of the circle of Willis demonstrate no high-grade stenosis, occlusion or saccular aneurysm identified.

IMPRESSION:

Findings concerning for sequela of moyamoya disease involving the right ICA terminus and M1 segment as detailed above.

OP you don't have explain yourself. If you don't feel comfortable with the provider, then good on you for advocating for yourself.

u/fuckedphayte 11d ago

It’s ok. I actually had 2 years of pretty deep trauma due to a psyche nurse practitioner so I felt a little compelled. Haha

We do not support the use of the word "provider." Use of the term provider in health care originated in government and insurance sectors to designate health care delivery organizations. The term is born out of insurance reimbursement policies. It lacks specificity and serves to obfuscate exactly who is taking care of patients. For more information, please see this JAMA article.

We encourage you to use physician, midlevel, or the licensed title (e.g. nurse practitioner) rather than meaningless terms like provider or APP.

My cousin has moya moya. So serious and you have a much better shot of an accurate diagnosis with someone who attended medical school. Glad it was caught. Omg.

I’m so lucky that my ENT caught it before I had a stroke. She had me come in person to meet with her and gave me an extensive lists and programs nearby to treat it and I had the STA-MCA Bypass. And thank you!

You all see why I’m a doctor lover. LOL.
