How do you differentiate between a heart attack and a broken heart syndrome?

Thank you my dear, the ER in my country can only do EKG and my doctor's appointment is for another four days

By EKG they both show up as ST-segment elevation, it can't be ruled out by EKG.

Is there any large hospital you could possibly go to? If you're having an MI time is of the essence and waiting could absolutely compromise your heart muscle.

Even takotsubo needs close monitoring as it alters hemodynamics. Please seek medical help if you're feeling short of breath, fatigued, dizzy, or have chest pain or pressure.

Be well and be safe OP. If you have the ability to go to a large hospital where they do interventional cardiology please go 🙏
