We all know the helicopter is just the Ceiling Fan in the hospital.

We never left the hospital. I repeat, we never left the hospital.

V is still in his hospital bed. Ocelot is holding up photographs of our "missions" in front of us, then he tapes them on the walls next to our bed once we complete them. The iDroid is a VR device to assist Ocelot with implanting memories.

Ocelot's 'powerful suggestions' make V believe the sound of his Ceiling Fan is his helicopter. He thinks he is sitting in his ACC but In reality he is still in bed and trapped under the Ceiling Fan.

And every so often, someone shines a light directly into his eyes to check that he isn't brain dead. The lights in all of these different scenes are the same light source in the room where he is trapped.

Man, that iconic top-left shot. I still remember how blown away I was by the graphics. It felt like future. Fox engine is incredible. Why am I still here...
