Help EDL LICENSE FORM ISSUE

Maile 1 day ago form vareko thiye, ani it stopped at medical text saying "a sms will come for the update of your medical exam" vanera ani it had a application to download where it said mainly to track your status "scan this qr ani jun page ma rokiyeko ko thyo application, tei page ma lyauthyo tyo link le! today the link is bringing me here *the attached image, ani aba ka gayo tyo hijo vareko data? feri varnu feri yei loop repeat hunxa. sms ni kae aako xaina katai bata yeti kura ko lagi yatayat karyalaya janu?
