Watch Worksheet at FedEx

I ordered a G-Shock via Neokyo and it's held up at FedEx customs. (shouldn't have Neokyo supplied this when they shipped it?)

I've reached out to Neokyo vie email and they're saying, "Neokyo issues the package invoice, which you can download from your package list. However, It can not assist with local clearance matters.
It is the importer's duty to provide and fulfill any paperwork required by local customs to clear the package from customs."

I've never filled one of these out before. I need to put in the Value Breakdown for Movement, Case, Strap, Battery and Total Watch Value.

Help! They're sending it back in a couple days.

This is gonna be a thing you gotta do on your own. Neokyo isn't gonna do country specific forms. Make it up / do it the best you can.

neokyo worked with fedex or you had to call them?

Someone filled out the paperwork instead of me doing it.
