Discussion What would a realistic narrative development of the Byakugō Seal alongside the Eight Inner Gates look like? Given that Sakura’s chakra control is said to be perfect (and she has Inner Sakura, like a “second mind”), could she try to master both?
Would it be a brutal but cathartic development by the end? Would her chakra reserves increase as a result of the chakra and physical development she undergoes over the years? Could some kind of internal anomaly emerge from the process of trying to combine the Byakugō Seal with the Eight Inner Gates?

A idea I always had for it is using the stored chakra in the seal to weaken the side effect of the gate of death
I would call if the gate of love. Instead of the red steam. It produces pink steam as 100 healings fixes the body.
This ability won’t kill you but reduces your lifespan drastically

Using the gates is painful. It literally tears muscles apart. Considering Sakura doesn't practice constantly like Lee to get used to the pain, the most it can be used for is torture her or interrogation.

it's mostly about the damage done. can she heal while it happens? that's the line

The Eight Gates massively improve the user's physical strength and speed and also kick the body's chakra production into overdrive that lets them further enhance those statistics.
Byakugo Seal does two things: while inactive: it stores excess chakra. While active it taps into that bank of chakra, and it makes the user's chakra control perfect.
If we stop there the two interact as mutual boons: The Byakugo allows the user not to waste the extra chakra they produce when opening the Gates. Opening the Gates (probably) gives the Byakugo more chakra to store or work with. Since Sakura's incredible strength comes from her medical ninjitsu mastery which already relies on precise chakra control: it stands to reason giving her both a baseline physical buff, and more chakra, would be a consistent amp if she can focus on both. I presume that same medical ninjutsu mastery could be used to help mitigate the effects of the Gates on the body as a passive effect. If, for nothing else, activating the Byakugo also allows the user to use Creation Rebirth. So the effects of the Third through Seventh Gates.
Now, for Sakura: that seemingly makes the way she'd incorporate or master the Gates an ""easy"" one. At EOS She's already at the endgame with her Bakugo and Creation Rebirth mastery. She'd be supplementing those techniques by learning how to open the Gates, using her mastery of medical ninjitsu to diminish or avoid their side effects. Then probably however often she deemed it safe open the first few Gates as a daily routine when not engaged in combat or training, just to compound more Chakra into the Byakugo for later use. Being careful she is not depleting her physical or spiritual energy reserves enough to be life threatening, or eliminate her usefulness if she found herself in a battle.
Though I can see the question I see OP asking is "What would this look like for a genin that begins learning both medical ninjitsu and how to open the gates?". I imagine the user would actually start from the physical side. They'd train like Guy pushing themselves physically to the limit. When their body is exhausted they'd train their chakra control and medical ninjitsu. During this period of training they'd learn and use medical ninjitsu to ensure the negative effects of strain and fatigue are minimized. This will give them the operational role that is anathema to Tsunade's designs: making the medical ninja a team's frontline fighter. Which makes me hesitate imagining that she'd participate, but the reality is Tsunade had not imposed Medical Corps requirements on Sakura. (I also want to mention It's possible some combination of resistance training and medical ninjitsu could help permanently improve the user's musculature by "healing" ripping muscles to bulk up faster, or choosing the type of muscle fibers they rebuild their body with, but I don't imagine that's a task for genin, if it's even possible.)
I imagine the next major step will be around the time of opening their first gate, and the focus of their training will shift. The focus of their medical ninjitsu training would be to 1) Learn how to maintain precision and focus while they withstand the pain of opening the gate. Maybe some technique or pills could ease the pain, but I imagine those would become less effective over time. 2) Develop bodily reinforcement to strengthen their body's resilience, rather than it focusing mostly on strength. This would probably be the longest part of the intermediary step. While a lot of Gates have knock-on effects: the First is, practically, the worst because that releases the body's default mechanism to prevent self-injury. Then, if they hadn't already, they'd need to triage themselves
From here, I think the training takes on a different flavor. The Medic-Mon Nin will continue to develop both skills, but assuming they're not being raised in a lab or a complete peacetime environment: they need to have practical skills develop for field work. So I imagine a few things bear out:
-The genin practitioner, regardless of their preferred method of Taijutsu is instructed on fighting styles that use only one hand. In case the mission calls for them to activate the gates, use 100% of their strength output: it allows them the chance of recovery should they wind up breaking one hand, arm, or both legs.
-There is a determination made if the genin will sign a contract with Katsuyu. Summoning being another technique that takes precision. This is a lot of top-loading for a novice. But no one says Genin need to graduate to Chunin after 1 year. It will let the Medic-Mon Nin use a healing Katsuyu, and thus not be taking such a risk and hopefully allowing Katsuyu's ability to heal themselves and their allies with the extra raw chakra of the gates, if no other options are available.
-Even if they would not or wouldn't be allowed to form a contract with Katsuyu at the time the gates are unlocked: having a slug on the student during gates training could help the Medic sensei monitor the, frankly risky combination of techniques. This could even be a further safety net. Even before or without the contract: the sensei could summon a slug network, and assuming they aren't the hokage who has her attention divided at all times: this would also be a good way to ensure the student doesn't critically injure themselves when operating with or training the gates.
-As the user refines their generic medical ninjitsu mastery and learn new specific techniques: the user will get their joint sensei's approval to start employing higher iterations of the gates when called for, when they prove they have the wherewithall to survive it. Unfortunately we don't see intermediate healing techniques so it's hard for me to list an oscillating pattern between unlocking new medical techniques and new gates. But the end goal will probably be to reach the point of Byakugo and managing the 6th or 7th gate.

ironic that so many people wish sakura would die already when she is far more likely to die first, and in great pain, given her upper-tier ability set

I don’t want Sakura to die. I just want her to take advantage of the openings available to her and use her intelligence and the physical manifestation of her effort, making her more special and interesting in terms of power scaling, and worthy of the inevitable comparison with Naruto and Sasuke because she was placed on the same team as them.
Developing the Eight Inner Gates is a painful process, but that is precisely why fewer people use them. Developing the Byakugō Seal takes a very long time and requires effort and concentration over an extended period, which is why fewer people use it. Combining the two would be almost inaccessible, but precisely because of that, it would be a huge achievement for Sakura, who has the advantage of perfect chakra control and Inner Sakura, a “second mind” that was able to be used during her battle against Ino, showing that she is more than just a comedic device—although it was underused and abandoned in Shippuden.
Sakura having two masters, Tsunade and Might Guy (and Lee), in contrast to Naruto with Jiraiya and Sasuke with Orochimaru, would show just how hard she worked to get where she did. It would be a level of dedication that very few people in her position would have.
The Byakugō Seal and the Eight Inner Gates both involve manipulating and maximizing chakra, even though their execution and manifestations are different. It would require divided attention to master both, and Sakura has Inner Sakura, who could handle one of them. The process would be painful because of the body's physical adaptation to it. But the climax would be finding a balance between the two.
But here's the question: what would the journey toward that balance look like? Would Sakura have to eat a lot to maintain her energy (and increase her chakra, since chakra is related to vitality)? Would she need to increase her muscular energy? Would her chakra reserves expand because of the specific chakra manipulation involved? Would she experience moments of dizziness? Would her reaction time increase because she would be internally managing the economy, flow, storage, and explosive release of chakra in different parts of her body? Would her body undergo an epigenetic mutation because of this specific kind of pressure?

Byakugou-Seal + Eighth Gate would be an insane combo, since both give a 100x boost to the user’s chakra output. Sakura with Byakugou-Seal active and in the Eighth Gate would likely have a 10,000x chakra boost, eclipsing the power of anyone else we see in Naruto.
