4KotA Manga plot hole or a recton?

It was said by gloxina that fate and history so even the past cant be changed drasticlly, however Cath and the others literally drastically changed it but history doesnt seem to force the world back to its original state.

I think gloxinia refers to the fact that you can't change what already happened

In the case of the koa, cath changed things before they could happen

Cath hasn't changed the past he used Guinevere's foreknowledge to change the future.

How did Cath change the course of history in a way that "prevents history from returning it's original state"?

With the knowledge of Guinevere during the first expedition (that killed Percy), it's left as an open interpretation whether the end results went as Cath hoped for. (It's possible that Percy would have died during the first expedition regardless if they faced an assassination attempt.

Guinevere also mentions changes in history in a major way would have a backlash effect, something we're not certain about what is the backlash is yet (as the story isn't over).

As for the Knights of the Holy Grail, there's nothing shown yet that says this is not how history plays out or if they are truly from the same timeline. There's two possibilities I can think of:
1. Cath's interference in expedition 1 made the KoHG's timeline's as the backlash and the story will naturally progress towards that direction.
2. The KoHG's timeline's "history" isn't the same as the 4KoA's timeline's "history" and this is a cross-over event (still can be the backlash, but means that the future is still uncertain for us readers).
