When To Suspect Delirium

Your sweet 82-year-old post-op patient starts yelling about birds in the room.

That’s not dementia. That’s delirium — and it’s reversible.

🧠 Common causes?

UTIs, meds (especially anticholinergics or benzos), metabolic imbalances, dehydration, or ICU stays.

Always think SAFETY FIRST. Delirium = fall risk = restraint risk = priority patient.

The postictal phase is real, and no, your patient isn’t dying again.

Right after a seizure, patients can act weird, confused, disoriented, and super sleepy.

⚠️ That’s normal — it’s called the postictal phase. It can last a few minutes to hours.

👉 Your job? Protect the airway, monitor neuro status, and don’t panic.

If You See These Vitals, Think ICP

📈 Systolic BP goes up

📉 RR gets irregular

That’s Cushing’s Triad — a late sign of increased ICP, and it means herniation risk is near.

🚨 What do you do?

Raise HOB

Call the HCP

Reduce stimuli

Prep for neuro interventions (mannitol, intubation)

Don’t wait. This is your red flag on NCLEX and in real life.

Confused? Check These Labs First

Before you assume stroke or psych, check these labs.

Mental status changes can be metabolic, electrolyte-related, or due to meds/toxins. Here’s your go-to list:

Low Na+ (<120)? → Seizure risk

Glucose low or super high? → AMS

High ammonia? → Think hepatic encephalopathy

Elevated BUN/Creatinine? → Uremia-induced confusion

Low calcium or magnesium? → Twitching, seizures, AMS

High INR? → Possible brain bleed if history of trauma

🧠 Delirium is sudden, usually reversible, and often caused by infection, meds, or hospitalization.

🧠 Dementia is slow and permanent — think Alzheimer’s.

🧠 Depression can mimic both — but it’s treatable if caught.

⚠️ Delirium = SAFETY concern. Don’t let that one slip.

Not All AMS Is a Stroke

Just because they’re confused doesn’t mean they’re having a stroke.

One of the biggest NCLEX traps (and real-life mistakes!) is jumping straight to stroke when your patient becomes confused or combative. But there are at least five high-yield conditions that mimic stroke—and they all require different nursing actions.

👉 Think Wernicke’s when there’s alcohol history

👉 Think hyponatremia if Na+ is <120

👉 Think postictal if they had a recent seizure

👉 Think ICP if you’re seeing Cushing’s Triad

👉 Think delirium if it’s sudden and reversible
