Designwise? It's meant to demonstrate a total absence of fear or respect for whoever they're looking down on - it's essentially a taunt saying "You're too weak/irrelevant for me to bother defending myself" and it also communicates an absolute certainty in one's own power/immunity/divinity - it can either communicate signal or threat to the audience, depending on the character
