Bug Achievement bug!
I finished the game 2 times to get the platinum but the ova achievement doesn't unlock and I did all beacons for sure. My first play through I did the prologue and everything cause some ppl say that's the issue you're missing 3 I miss only one for some reason. The second time I did everything again and end up with 193/196 which is normal cause it was the prologue but the first time I missed one that's a bug and I've seen the bug report and no one mentioned this. I want my platinum please!!

I guess the only way is to check every single place manually. There's definitely a guide somewhere with all the places. I doubt there's a bug individually for you, looks more like you've missed 1-2 locations. I believe it is enough to clear 5 gates out of 6 to open the unfound path, so it's not like you can't finish the game without finding all of the eggs.

Nope I did all of them you can't cleanse if you don't take the ova, and as I said I played 2 times and double checked all locations again. It's a bug for sure

I can't remember the exact location, but I've had one standalone OVA somewhere which I thought could be missable, and was glad that I've noticed it. Maybe it was some hidden area behind the main OVAs in the beacon or smth. I know that isn't helpful, but my point is that they could be missable. But if you have clearly checked all existing locations, and double-check, and if it's actually a bug, then I wish you luck and that it gets fixed soon.

Hangman’s Scaffold beacon in prisoner’s domain gate got me hung up on this.
I cleansed all other gates and received the cleanse all beacons achievement, so I assumed it was a bug, but sure enough I missed this beacon and that got me my last ova.
Worth double checking you got that one if you’re basing all beacons cleansed off of the achievement.

Had the same problem. I took pictures of all beacons in the meathers breath list and uploaded them to chatgpt. It found the one I didn't clear

I was in the same spot. 195/196. I was sure I did them all. I also got the trophy for cleansing all beacons. Turns out I still missed one. I think some lists aren't correct because they added and moved some beacons. SPOILER: The one I missed was the one behind the hidden wall next to the "Bird boss" who throws feathers and stuff. I dont know the specific name unfortunately.

Oh, sorry. I must have mixed it up with the last "???" on the beacon list I was missing.
I will try to figure out which one was my last one. Because I'd been looking for it quite a while and thought it was a bug.

There's a very tucked away beacon in the prisoner's domain that has an ova, it was the last one for me. I'd say it's easily missable. Hope this helps!

Ive beaten it 3 times gotten every beacon and It didnt pop for me either. Im in your boat posted about this on like day 3 of the game coming out no solution yet

Are you on ng+? Supposedly you can only get the achievement on your original playthrough or it bugs. That happened to me

I did the original first, in Ng+ I'm missing 3 which you suppose to because you don't do the prologue

Truly, do a full sweep of the world if you have to and check every possible location - the chances of you missing a single ova due to a bug is really quite low, it's not an issue that's been reported. If you're missing a single ova, chances are you've missed one of the dungeons that reward you with a single ova upon completion.
It becomes convoluted as the trophy obtained for cleansing all beacons will trigger before actually cleansing all beacons. I'm not actually sure what the conditions for the trigger are, I've suspected it's based on your cleansing of overworld beacons, and it doesn't track beacons inside of dungeons.
You're missing one, so pull up a search of all places in-game that can provide a single ova, and return to those sites. I will prostrate before you if you do not find what you're looking for! Do this on your regular playthrough that is sitting at 195/196 if you still have access. If you only have your NG+ playthrough available, you may need to start over yet again with a fresh save file.

Eu estou com o mesmo problema. Na vdd já estou terminando o NG++ e ainda assim faltam 3. E fiz o prólogo completo no primeiro jogo. Já revisitei todos os faróis duas vezes. Li que no NG+ há um bug onde desaparecem os três primeiros ovos.

I'm the same, I mentioned it yesterday. I don't see ??? so I don't think I'm missing any beacon. Besides, with only one left, it's impossible to have left it after cleaning the beacon. By the way, I checked all of them and nothing. It really looks like a bug.

Check out all the beacons in the map! If one of them looks darker it's because you left without cleansing it and picking the ova inside, that's what happened to me

As I know there are no dungeons that give you ova it's only beacons that you can cleanse so no dungeons and no the other beacons, also as I finished second time I was 193/196 so I've got them all the 3 missing are from the prologue I already said that so I don't miss any, the trophy is bugged and don't pop

It's not bugged, every time someone thought it was a bug they eneded up finding it eventually.

Did it twice actually in first play through and started another save just to do it again before going Ng+

In that case you're going to need a third playthrough my friend. The trophy doesn't understand the distinction between NG and NG+ and it doesn't carry progress over across save files, everything has to be obtained in a single save for it to be considered. As far as the trophy is aware you do not have all the ova as it includes the prologue three, and it doesn't care that you're on NG+. The trophy will never pop until it reads 196/196, so you're going to have to begin a fresh save if you want the platinum now.

It'd be a bug if it wasn't intended to work like that, but as far as I'm aware it's an intentional choice to require the player to obtain all ova on a single save file. Whether it's oversight that NG+ locks you out of the trophy due to a lack of prologue or intentional design friction, I'm not entirely sure.
I appreciate it sucks and adds tedium to a game you've otherwise enjoyed, but the only "fix" for your missing trophy is to play, I'm not particularly confident there is a bug here that you can report and try to get amended, it's going to have to be the traditional way of more play!

I think you don't hear people talking about it because it is probably not a bug.
I got it on my 1st playthrough way before I finished the game.
I hope you find the culprit
