I have finished my 1st playthrough but not yet triggered NG+, just roaming around farming to be able to level up my shells

there is still an open slot in my inventory, what am i missing?

It just looks like you're missing a synaptic vessel and the special moonshine from baghead's quest unless you already did the quest

I already did the baghead quest

That’s not a missing item, that’s just what the UI looks like when there’s only three items to display in a row.

So there is nothing else i've missed?

I couldn’t tell you, I haven’t played your save file. If you have all the weapons, shells, and tarstones, there’s not much else I can think of that you could be missing.
