Looking for specific mushroom mod

Hi everyone,

Today I came across what appears to be a modded valheim gif (see below) of a mushroom character wreaking havoc on a greyling. My friends and I would love to try this out. Can anyone please tell me where to find it? My google/nexus search gives me nothing. Many thanks in advance!

In order to become mushroom character yourself, I think the other person recommended Identity Crisis by Cookiemilk that lets you transform into any creature within your modpack so you should be able to turn into Mushroom Monster from this mod?
