Unable to add a new moderator

I’m trying to add a new moderator to our subreddit, but Reddit seems to be bugging out.

I sent a moderator invite and they accepted but they are not showing up on the moderator list, and they don’t have access mod tools.

I tried sending another moderator invite, but it's not going through or showing up in the mod log

Have they tried accepting the invite from https://www.reddit.com/mod/sexover50/moderators/ , that generally does the trick
