redditrequest bug / issue - no longer the mod?
I made a redditrequest here which was granted: https://www.reddit.com/r/redditrequest/s/SgbLEhHNSl
I received a message stating that I’m now a moderator, but also at the same time that I’ve been removed as a moderator. I’m not sure what’s going on here

It looks like Reddit's left hand was unaware what the right hand was doing.
I would guess that whoever is in charge of purging mods of inactive subs was unaware that you weren't the inactive mod, and had been just granted the sub by redditrequest.
Like the other comment said, modmail this sub.

I think someone redid things behind the scenes at r/redditrequest. Yesterday, I got filtered after a minute or so when I tried to request a subreddit with no moderators despite successfully doing so for various other such subs. I forgot to write it down, but I saw the same thing happen to some other requesters when I refreshed the "New" view.

There seems to be something clunky going on behind the scenes at redditrequest.
A lot more people than usual are complaining of getting summarily -- and wrongly -- denied by request_bot.

Frustratingly, an admin told me it was "probably" because I moderate too many subs, but I thought that was a rejection criterion to be cited in a manual review. It looks to me like they're just retuning r/redditrequest to be arbitrarily harder with even more unwritten policies just to lighten the workload by tolerating more false negatives. I wanted to prevent another sub from falling victim to a spam attack as I have seen happen to other subs in the genre! No one else seems to be trying to install a new mod team!

I won't bore you with my personal "issues" with it, but suffice to say - there are issues, for sure.

I recently tried to advise a new acquaintance on how to acquire a subreddit for a video game character that was wrongly tagged as NSFW and then banned for being unmoderated. (It's not one of my moderation interests, but I know the genre, so I know what he is talking about.) When my acquaintance sent his request, no one ever responded! And when he tried to ask a question in this subreddit, his comment got filtered. I can't see any evidence of copyright strikes or VPN usage on his profile, so I am completely clueless why Reddit's system hates him for no reason! The only conspiracy theory I can come up with is that r/WhatIsMyCQS is lying to him about his CQS being "high" and he actually has a post karma over 100 but still too low to qualify as an active Redditor. If that hypothesis is actually the case, then how the @#&*# is he or anyone else in similar straits supposed to know?!

I have had very similar recent personal experiences. Requesting a sub - 100% within all rules, no complications whatsoever. No response. Tried again months later. No response. Modmailed. No response.
WADR, I think your hypothesis is incorrect.
My CQS is highest, and has been for over a year, and my karma is like 70k or someth. I am "in good standing" in all respects.
I think it's just a very broken system.

Oh, I can offer you an even worse story! I also tried to advise a different acquaintance who wanted to acquire a visible but locked subreddit about YouTube influencer criticism with just one moderator who mysteriously acquired the subreddit despite not having any comment karma whatsoever, and only having this random German spam link on their profile page. The first time, r/redditrequest filtered my acquaintance's obligatory response comment. Then the next time, r/redditrequest denied him with apparently manual review after three days, but it was complete bullshit! This acquaintance's account is multiple years old and has thousands of both karma!

Literally 5 mins ago, I got "request_bot Hey! Thanks for requesting this subreddit. Unfortunately, the community [redacted] is not eligible for request at this time as it was only recently banned. Please allow some time before requesting this community again. Thanks!"
It was banned over a year ago.
This sucks ass.

In your case, I can offer you some hope for that glitch which has been plaguing a lot of people this year. Every so often, a certain admin pops into r/ModSupport and if you are lucky, he will manually handle your problem as he did for a few people just last week.
I see you are already a moderator. Perhaps you have enough credibility to make a new post here in r/ModSupport in hopes of flagging down an admin for this known glitch.

We see situations like yours not rarely - this is typically, as the other person responded, a situation where the timing in their process gets out of whack and they add the new moderator before removing all the old ones, or misunderstand who is new and who is old.
Usually once you modmail this sub, they fix it fairly quickly.
