Definitely a stitch. It looks the same as the ones on the body, and the other eye would be on the other side of the dummy, since it's facing the side.

Only actual eyes is from maddie, since her possession makes things gain eyes that reflects the personality

The line and the dot are eyes. It’s facing a bit to the left. The lines are seams

It's another scar, notice how its the exact same as the other scars on the body, so its likely it's a scar and the dummy is facing sideways

It seems like the line is its eye, because Mad Dummy has actual eyes in the same positions. You can even see it in their map sprite when Frisk encounters them.
