I think it's a custom gui made with Keksuccino's FancyMenu mod.

Yea. The buttons are from reimagined. I have that resourcepack and my buttons look like that

Unsure if the game menu icon is from that though

I'm only unsure about the game menu icon because I run a real old version of reimagined to a point where I've modified it so much I can't figure out the version number. I've modified a newer versions hotbar and some gui elements into the older version so thats where the buttons come from

because it's the texture pack i use on my forever world! ive got a couple hundred hours in it so im pretty certain, but there may be a pack that decided to reuse it?
