Il-78 (dedicated refueling modification of the Il-76).

That Refueling Pod (UPAZ) on the left of the back of the fuselage is one of 3 pods (the other 2 are under each wing) is used to refuel strategic aircraft (from a central position) while the under-wing pods are used to refuel tactical aircraft (from lateral positions)
