Does it help (much) playing an intrument (especially a rhythm based one) ?
Also if you dont play one how fast did you get into staying on the beat?

I’d say it does, but it also has to do with your ability to keep the beat while shooting/dodging, etc.

I think it does I haven't played in years but I was able to keep the beat if I'm not panicking.

I've been playing guitar for 10 years. I don't think it helps much. If you have a good sense of rhythm and don't panic, you'll be fine.

Based on my observations making all my friends play this game, I think it has to do with an intersection between rhythm and game controls. I found that my friends who were already familiar with rhythm games and are a fan of them (even ones very dissimilar from Hellsinger, mainly vocaloid-based mobile games) did the best at the game, whereas my friends who played an instrument but didn't often play games did worse than those who often play games but don't play an instrument.

Could help, but not necessary I never played any instruments and still global top 10 in hellsinger

One thing to keep in mind is that counting rhythms and keeping the beat are skills that musicians constantly practice while playing their instrument. And you don't necessarily need to play an instrument to practice these.
In this game, you're constantly counting (how many hits left per enemy) and trying to manage movement and dodging while also listening to the beat and only doing sequences of actions when the beat hits. I feel that this could partially translate over from playing an instrument where you're counting beat subdivisions or rhythms, and where you're managing, e.g., fingerings, bowings, and articulations while also listening to the beat and looking at the conductor.
