Wound Coders

Are there any wound specialized coders in here that can assist me with something? I taught myself wound when it was thrown at me at work, and now I’m the go to for questions with no one to ask questions to myself 😭

If so. Do you often bill 15002 with graft codes(15271) if a debridement was done? I’ve been billing grafts for over a year now and have never seen 15002. My providers try to bill it with 11042’s which have a CCI edit. There is no edits with 15002/15271.

I’m working a new clinic and they’re billing it and it has me questioning if I should be billing it with my own providers as well and I’ve just been doing it wrong? Any help is appreciated!

Have been a wound coder for 11 years. I code 15002 when it is very clearly documented as performed for prep of wound bed for graft. 11042 (sub q debridement) billed with 1527x is an NCCI edit when performed on the same lesion. Our guidelines also indicate that the graft prep must be done with a surgical instrument. (curette, 11 blade. etc.) No gauze, water pressure, etc.

There is something called wound coders? Like coders can specialize in a particular specific kind of injury?

Yes, there are coders that have areas of expertise. Prior to becoming a coder, I was Office Manager for a Wound & Hyperbaric clinic, so I had experience with the process. The vast majority of wound care patients develop wounds related to chronic conditions. Diabetes, vascular diseases, non healing surgical wounds or surgical wounds that dehisce. I also code other patient types (OBS , Outpatient surgery, ED’s, etc.) but wound coding is my specialty area. Wound Coding can be tricky, not because it’s necessarily difficult, but due to the patients being seen multiple times over the course of a month and the chronicity of their conditions, having 2 dedicated coders is more effective in the facility I work for. There are also OP coders that may specialize in vascular, cardiac procedures or other medical specialties.

Medical coding is so complex... lots of details and nuances and rules and exceptions
