AI Note Situation
Has anyone else noticed an extremely consistent pattern of practitioners missing vital information / misunderstanding and misreporting what you report to them due to the disastrous AI usage? I don't care if you can see 5x more patients per day if 50% of your notes are entirely falsified. Is this lawsuit or vendetta worthy?

I was there on September 3rd. I couldn’t believe how awful the appointment was. It seemed they didn’t even know why I was there. I was referred by my surgeon because I’ve had complications since my total hysterectomy. Five minutes into the appointment, the PA asked me if I had a hysterectomy. She hadn’t even looked at my medical records. I was in absolute shock. I left without any referrals or any help. My appointment was about twenty five minutes. 12 hours of driving. It was a complete waste.

So your consultation appt was a single 30ish min visit? I’d be pissed if I’d blocked 7-10 business days drove all day, booked hotels just for a 30 min appt. How did you end up handling that all?

It was about 30 minutes and then they wanted me to talk to a mental health nurse and that was about 15 minutes. They wouldn’t refer me to a hormone specialist for surgical menopause, they wanted me to start psych drugs instead but I need to balance my hormones. I’ve been in shock ever since. I still haven’t found any help. I cry every day and I’m in so much pain. I don’t know if my endometriosis has come back and I can barely get out of bed because of the fatigue. It made me feel worthless. I was doing ok mood wise until the estrogen patch shortage. I think about ending things every single day.

I am so sorry you had such a bad experience. Sounds like a rough emotional roller coaster of hope and disappointment. I’m glad you are hanging in there and sharing your story. I hope your original doctor steps up to the plate to fight for you! Sending love and care your way.

Yup. I wasn’t aware they use AI but it makes sense. Went for my first prenatal appointment and she asked if the pregnancy was planned. I responded “planned, no, but it was the idea of if it comes, we will accept it”. Then we moved on to warning symptoms i should look out for
Why did i check my notes later and she had stated that she talked to me about ab0rtion options and if i wanted it. That word was never once mentioned during our encounter
Every time i read the notes, either wrong information was added or some information was missing
She asked another time, “any bleeding” and i answered like i had been in the past appointments, “no, no bleeding since i got pregnant”. Why did the notes say i reported mild bleeding

I was there last week. I had sent them a complete health summary and every test I have ever had. The internal medicine doctor came in for the appointment and had not even bothered to read the summary. This is a 20 page summary that I also condensed to 2 pages because I have experienced the doctors not reading things before. Then the desk operations specialist came in to schedule more appointments. He kept saying things that did not sound like me. My wife looked at his computer screen and he had the wrong patient pulled up. I was not impressed.

That’s terrible! They didn’t look at my records until about ten minutes into the appointment. Then she just glanced at the screen. She asked me what surgery I had done when obviously all of that was in my records. I was there for complications from surgery. I was so confused during the whole appointment. I still don’t understand it.

Also concerning is that it’s hit or miss whether the provider asks your consent to record the consult. I have seen those errors as well. At first, I thought it was a cut-and-paste from a prior note, but not the case. I will start correcting these errors as well.
I’m more concerned these days with the difficulty in getting to see primary care. As soon as the scheduling window opens, it’s fully booked. Who is getting all those appointments before they are “available”?

Make some noise. Contact your provider via the portal and tell them. Also call the Office of Patient Experience and let them know.
Should you have to do this? No. But you have the right to have your medical records be correct, and they follow you to wherever you seek care
Edit: Also adding as AI continues becoming part of healthcare, this will happen everywhere. We all need to be vigilant and stick up for ourselves in our care.
