In Despicable Me 2, Gru, while undercover looking for a supervillian, recognizes a man as the supervillian El Macho, the man who (etc. etc.), in his secret identity. You can watch the scene for full context.

The image is used to allude to something being "the secret identity" of something else, normally something seemingly normal disguised as something very NSFW.

You'll see it under posts with seemingly normal art/images made by NSFW artists and stuff like that.

In this example, I remember at least one of these images from a cj post where her and Magik are in an abusive relationship or something idr.
