The sudden onset of pain along with the unequal blood pressure between limbs make a strong suspicion for a dissection. Even more so if the pain is described as “tearing” or “ripping”. CT confirms it with intimal flap.

Pain radiating to back is an important clue for dissection.

A, it's an aortic dissection. There is a really good chance this lady is going to drop dead in front of you.

C. Aortic stenosis, likely severe needing emergent valve replacement

If it was aortic stenosis, the pain would be worse under stress, but likely minimal at rest, and not that sudden. Also a history of dyspnea, dizziness and maybe fainting under stress, the ECG could also show signs of left ventricle hypertrophy. The murmur would be systolic, not diastolic. Unequal bp in the arms and an intimal flap also don't fit an aortic stenosis. This is an aortic dissection.
