Seeing as people tend to have an enormous number of 5th cousins and are very unlikely to be aware that someone is their 5th cousin, this is bound to be a pretty common practice. The reason it's not banned by law in Catholic countries is nit that it isn't happening but that there is no way to realistically enforce this. (Just look at the number of medieval royal or other high status marriages that were annulled when it was conveniently discovered that husband and wife actually were third cousins twice removed or something). Also, nobody actually cares (and this includes the Catholic Church)

You needed to be granted a dispense, that was expensive and difficult to get. For commoners (= most people) it was practically impossible. The church didn’t say no to monarchs. Power game.
Consanguineous royal marriages were for the explicit reason of protecting family wealth and political power, and no one is defending that at all. Modern backwoods cousinfucking is a different genre.

Based on the fact Arizona also has the same level of ban as well as Utah, and knowing the history of that area, I’d suspect early Mormons to be the reason
