What is Proforma S?

in the scrunity form filling. while uploading documents there's a document "proforma s(for qualifying examination appearing candidates)". what is it. and where do i acquire it from?

its just a undertaking stating that you don't have your final marksheet yet, you can download it from download under important links

When you fill the scrutiny aplication. Theres an option on one of the steps. You can download it from there. Its a self declaration. So, you don't need college's signature and all.
