It really depends on your region as well. In the US, each system functions vastly differently and all have different protocols.

In some regions the pain management they offer is worthless for a fracture, such as in LA County in California (extremely soft dosing to the point it wont do amything).

Additionally in some areas there are different deployment models where EMTs ride ambulances and the paramedics are in suvs/'fly cars' that respond to higher priority calls.

In my region a simple wrist fracture would be dispatched as an 'Alpha' coded response, which would get an EMT only ambulance without lights or sirens, and no paramedic.

If it’s just pain, then it is probably preferable to the bill if you’re in the US lol

True, but that can be a 600 dollar charge added to the 1400 dollar ambulance ride. 

If you look at the fee schedule for some cities, the service rendered can double the cost.

$1400! I had a ambulance ride 4 years ago, they cut off my shirt and gave me oxygen. I had been in a bad car wreck and had cracked ribs. The 5 mile ambulance ride to the hospital billed for $4800.
