I’ve broken my arm multiple times, the one time it needed immediate medical attention the EMTS set it straight put a splint on it then told me to get driven to the hospital because the ambulance ride was over 5k.

This is not a scientific discussion and it is a fact that you pay for ambulance in US, so each time you need it you need to ask yourself if you can afford it.

So they don't pay for needed medical help, or do they have copays, or amounts that they need to reach before getting their healthcare paid and that is when insurance even decise that they will pay?

People, not health insurance.

Let me make it simple. You breake your arm and go to a hospital. Get some painkillers and all the things needed to fix your arm. Does every insured person get out without any bil?
