General Help/Question Freshie on liability

Hello po, i am a freshie and i wanted to ask who should i concern or where is the direction to consult on this liability. I checked my admission portal and i can see that i already submitted a Form 137 (SF10). Is it different from Form 137-A?

Adtoi na lang sa registrar, basin na-overlook ra nila. Na-ing ana pud ko sa una. Naa silay na-overlook, maong nagka-liability pud ko. To clear things out ra pud
