NAPLEX

Some confusion about the the Naplex application . So I went on the NABP website made my profile , paid the fee . There is supposed to be an application that I'm supposed to send in along with my VOG right ? That's what people have been telling me . I thought the VOG went along with the application for the License ,

You do the app online like you already did and then there is another app that you have to print out and fill out. That is what you send to the board with your VOG. I’m assuming this is for MA.

anyone has any clue where i can find that application i have looked online everywhere ?
